I know that you did not want a surgeon in the US due to cost, but there was recently an article in the Phoenix Sunday paper about a surgeon at a famous neurological clinic here in Phoenix who does the type of surgery you mentioned. I don't know how this forum works, don't know if I can put a name. I would be happy for you to email me, but don't know if my email address would show in the post. This surgeon is evidently very very good.
