My Dad has had similar heart difficulties. What we finally ended up doing was contacting someone at Vanderbilt and having them look over his medical records and devise a treatment plan. He is MUCH better. Find a good teaching hospital in your area and email someone in the Cardiology department. See if you can get your son in so that specialists can thoroughly look over his symptoms.
